HAQM Clinic customers can access their profile and treatment plans by logging on to HAQM One Medical Pay-per-visit here. Customers who have additional questions about their profile can contact our customer care team at 1-888-663-6331.

For customers who declined the Health Insurance Portability and Accountability Act (HIPAA) authorization, HAQM will delete their data, and they will need to create a new profile should they return for future HAQM One Medical visits. However, these customers can access their records by contacting their treating provider group directly. HAQM Clinic customers who did not decline the HIPAA authorization can request that their health data be deleted by contacting our customer care team at 1-888-663-6331. Like all healthcare organizations, we are required by law to retain some information in order to comply with state/federal records retention requirements.

Not by default. Customers and patients may elect to share their information with HAQM One Medical by purchasing an HAQM One Medical Pay-per-visit service and agreeing to the Medical Terms of Service. HAQM Clinic customers who choose not to use HAQM One Medical will not have their data transferred to HAQM One Medical Pay-per-visit providers, and if they choose, can request that their HAQM Clinic information be deleted by contacting our Customer Care team at 1-888-663-6331.
